In Instinct EMR, the Procedures widget on the patient snapshot gives providers immediate access to a patient's procedure history without searching through past visits or Ax Mode records. Procedures performed through Instinct's Ax Mode populate automatically, and historical procedures from other hospitals can be added manually.
What the Procedures section shows
The Procedures section lives in the Chart tab, within the Snapshot page.
Procedures displays the patient's most recent procedures in a scannable list, including:
Procedure name
Date performed
Source (Ax Mode or manually added)
This section shows the 5 most recent procedures by default. Use the search bar within the Procedures section to find a specific procedure, or paginate through the list to see older records.

How Ax Mode procedures appear automatically
Any procedure completed through Ax Mode in Instinct EMR will appear in the Procedures widget automatically—no manual entry needed. These procedures are labeled with their Source as Ax Mode to indicate their origin.
From a listed Ax procedure, click the three-dot menu (⋮) on the procedure row to:
Open the Ax Sheet Setup panel
Navigate directly to the Ax Sheet
Add a historical procedure manually
Use manual entry for procedures performed at another hospital or before your clinic adopted Instinct.
Open the patient's chart and go to Chart → Snapshot.
Locate the Procedures section.
Click the three-dot menu to select Add Procedure History.
Enter the procedure name, date performed, and optionally add the practice name and any pertinent notes.
Click Save to add it to the Procedures section.
