Set Up Automatic Charges for CRIs and IV Fluids

In Instinct EMR and ITP, automatic charges can be attached to constant rate infusions (CRIs) and IV fluids, so fees are invoiced at a set frequency the moment an order is placed, with no manual entry required from staff.

Set up automatic charges for a CRI

In Admin, you can create automatic charge products and attach them to a CRI. When the CRI is ordered on the Tx Sheet, the attached charges are automatically invoiced at the configured frequency.

Common CRI automatic charges include:

  • CRI Set Up Fee (Once)

  • CRI Administration/Monitoring (1–12 hours)

  • Syringe Pump Usage/Monitoring (1–12 hours)

To attach automatic charges to a CRI product:

  1. Go to Admin → Inventory and open the product you want to configure.

  2. In the Automatic Charges section, select + Add automatic charge.

  3. Select the automatic charge product and set its Frequency (for example, Once or q12h).

  4. Repeat for each charge you want attached, then select Confirm.

Once the CRI is ordered on the Tx Sheet, it appears under the Continuous Infusions section with a dollar sign icon indicating automatic charges are attached. The first charge lands on the invoice at the order's scheduled start time. For example, if the current time is 12 PM and the order starts at 2 PM, the fee appears at 2 PM.

Note: To use automatic charges, you must first create automatic charge products in Admin with the class set to Automatic Charge. If this class is not available in your product list, contact support.

Capture CRI medication and volume using the One-Off feature

If your practice wants to capture the specific medication and mL volume used in a CRI for record-keeping, use the One-Off feature to log the medication alongside treatment notes. This is separate from the automatic fees associated with the CRI product.

If your practice uses Cubex or Omnicell, medications and mL volumes are captured in your practice information management system (PIMS) at the time of removal and automatically downloaded to the Instinct invoice. (Instinct ITP only—see How Cubex/Omnicell Work with Instinct for details.)


Set up automatic charges for IV fluids

IV fluid charges work the same way as CRI charges. A shift charge covering setup, administration, monitoring, fluids, and supplies in a single fee reduces the number of line items on the invoice and keeps things simple for clients.

Common IV fluid automatic charges include:

  • Fluid Pump (Once)

  • IV Fluid Monitoring (q12h)

  • Extension set/admin set (Once)

Once IV fluids are ordered on the Tx Sheet, the fluid order appears in the Continuous Infusions section with the dollar sign icon, and automatic charges populate on the invoice at the scheduled start time.

If your practice needs to capture each new IV fluid bag for inventory purposes, use the One-Off feature to log when a new bag is opened.


Frequently asked questions

Can I attach more than one automatic charge to a single CRI or fluid product?

Yes. Multiple automatic charges can be attached to a single product in Admin. Each charge can have its own frequency (for example, once for a setup fee and q12h for ongoing monitoring).

What if the CRI medication charge is already included in the automatic fee?

If your practice incorporates the medication cost into the automatic charge fee, Instinct will not separately capture the mL volume used. Use the One-Off feature or treatment notes to document the medication and volume for record-keeping.
