In Instinct EMR and Instinct Treatment Plan, monitoring templates let practice admins pre-build monitoring sheets for each procedure type, so a new sheet opens with the right vitals already in place. This article explains how admins create and manage templates, and how any user starts a new monitoring sheet from one.
Before you begin
To create, edit, or delete templates, your user role must have the admin_monitoring_templates permission. Roles that have admin_axmode automatically include it.
Any user can select a template when starting a new monitoring sheet. No extra permission is required.
If you don’t see the Monitoring section in your app, contact support to discuss enabling it.
Create a Monitoring template
Go to Admin → Medical Record → Monitoring (previously labeled Ax Mode).
Click New Template.
Name the template, then select a monitoring type: Anesthesia, Critical Care, Dialysis, Sedation, or Transfusion.
Click Save. Multi-location practices must choose a practice location before clicking Save.
Once saved, you can search for and add the vitals you want on the template.
Drag and drop vitals to set the order that they will appear in on the Monitoring sheet.
Vitals and their order automatically save to the template, and the order carries over to every sheet created from it. You can create multiple templates under each monitoring type.
Note: The five monitoring types are standardized. You cannot rename, add, or remove them. |
Edit or delete a Monitoring template
Every practice automatically has a default Anesthesia Sheet template that you can edit or leave as is.
To edit any template, go to Admin → Medical Record → Monitoring, select the template, and update its name, type, or vitals.
To remove a template, go to Admin → Medical Record → Monitoring, select the three-dot menu to the right of any template, and choose Delete, or click into the template and choose Delete in the top right corner.
Note: Deleting a template only removes it from the list for new sheets. Monitoring sheets that have already used the template are preserved and can still be viewed. |
Start a new monitoring sheet from a template
From the patient screen, click Monitoring and select New Monitoring Sheet.
Search the template list by keyword or filter by monitoring type.
Select a template and click Create Sheet. The sheet opens with the template's vitals already added.
Enter any procedure details in the monitoring sheet setup panel, which opens by default.
The template name and monitoring type display in the sheet header throughout the case. To view past sheets, click the monitoring sheet title in the header and select a previous sheet from the drop-down.
Note: For practices with Monitoring enabled, the Ax Mode button is now named Monitoring, and Ax History is now Monitoring History. |

Frequently asked questions
Does every practice get a default template?
Yes. Every practice has a default Anesthesia Sheet template that works like the previous Ax sheet. The only difference is that you now select a template when creating a new sheet.
Are templates shared across practice locations?
No. Templates are scoped to each practice location, and each location manages its own templates.
Do I still need to add vitals as Other Orders & Monitoring orders?
Templates add the vitals for each procedure type to the sheet at the start, so you no longer need to add them as separate orders, but the option is still at the bottom of each Monitoring sheet. If a vital is missing from a template, ask your admin to add it.
Does a template change how I record vitals during a case?
No. Templates only affect setup. Recording vitals, adding notes, and managing the sheet work the same as before.
