In Instinct EMR and ITP, hospitalization charges can be automated directly from the Tx Sheet, reducing manual entry and ensuring accurate invoices throughout a patient's stay. Charges begin when you select a hospitalization level—or at a configured start time—and continue until the patient is checked out or the level changes.
Select a hospitalization level on the Tx Sheet
To start charging for hospitalization, select a level from the drop-down menu on the Tx Sheet. Instinct will invoice the charge at the frequency assigned to that level and continue charging until checkout or a level change.

Open the patient visit and select the Tx Sheet tab.
Click the Select Hosp drop-down menu in the top toolbar.
Select the appropriate hospitalization level. Instinct begins charging at the time of selection, unless a preferred start time is configured for your practice.
Tip: Include a time range in each hospitalization level name. For example, Hospitalization Level 1 (1-12 hours). This sets client expectations upfront: a patient hospitalized for 3 hours or 10 hours pays the same shift fee, and the name explains why.

Configure a preferred start time for hospitalization charges
Instinct can be set to begin hospitalization charges at a fixed time each day rather than at the moment of selection. For example, if charging is configured to start at 8 am and the hospitalization level is selected at 6 am, the first charge lands on the invoice at 8 am. If selected at 10 am, the first charge lands at 8 pm.

Note: Hospitalization charging by patient weight is not supported in Instinct. If your practice currently uses weight-based hospitalization tiers, we recommend transitioning to shift-based pricing before go-live.
Start hospitalization charges automatically at check-in
Practices can configure hospitalization charges to start automatically when a patient is checked in. This removes one step from the admitting workflow and prevents missed charges during busy admit periods. Patients checked in as Outpatient will have hospitalization charges begin at the time of check-in.

Frequently asked questions
What if I select a hospitalization level mid-shift?
If your practice has a preferred start time configured, the first charge applies at that scheduled time, regardless of when the level is selected. Without a preferred start time, charging begins immediately at selection.
Can I change the hospitalization level after a patient is admitted?
Yes. Selecting a new level from the Select Hosp drop-down on the Tx Sheet immediately updates the ongoing charge to the new level. The previous level stops charging at that point.
Should I pre-enter hospitalization charges in my practice management system?
No. Because Instinct operates as a real-time Tx Sheet, pre-entering hourly hospitalization charges in your practice information management system (PIMS) can create duplicate entries and complicate invoice auditing. Enter hospitalization charges directly in Instinct at the time of admission.
