Preview and publish a form
Go to Admin → Practice → Forms and open a draft form.
At the top of the form, select Preview. The preview behaves like the live form.
Publish the form when all edits have been completed.
Review the summary of sections, total questions, and required questions.
Confirm the publish by selecting Publish form, or select Go back to make more edits.
If publishing fails, the builder flags errors on each question, grouped by section. Fix each error and publish again.
A form with the Enable account creation toggle on will be blocked from publishing until the following required fields are added: Client Name, Patient Name, Species, and at least one of Client Email or Client Phone.
Warning: A published form is locked and cannot be edited. To make changes, duplicate the form and make edits before publishing. |
Duplicate a form or fix a mistake on a published form
Go to Admin → Practice → Forms.
Select the form you want to edit.
At the top of the form, select Duplicate.
Open the new draft, which is named “Copy of” followed by the original form's name.
Make your changes and publish the copy.
To replace the original, archive it.
The copy is independent. Editing, publishing, or archiving it never affects the original.
Archive or unarchive a form
Go to Admin → Practice → Forms.
Select the form you’d like to archive/inactivate.
At the top of the form, select Archive.
To unarchive, filter the Forms page to archived forms, select the form, and select Unarchive at the top of the form. The form returns to its previous status.
Warning: Archiving deactivates every link already sent for the form, including links that clients have not completed. |
