In Instinct EMR, the Document Audit Log gives staff a complete record of every change made to a chart document—including who made the change, what was changed, and when. This is useful for maintaining accuracy, accountability, and compliance in patient records.
View the audit trail summary at the bottom of a document
Every chart document displays a brief audit trail at the bottom of the page. This summary shows the most recent activity on the document, including:
Created By
Edited By
Completed By
Signed By
Uncompleted By
Addendum By
Each entry includes the staff member's name and the timestamp of the action.
View the full audit log for a document
To see a complete, chronological record of every edit made to a document, open the full audit log.
Open the chart document you want to review.
Scroll to the bottom of the document.
Select View Audit Log.
The Document Audit Log panel opens and displays all changes from most recent to oldest. Each entry shows the staff member's name, the action taken (such as edited, completed, or renamed), the specific field that was changed, and the timestamp.
Note: The audit log is read-only. Entries cannot be edited or removed.

What actions does the audit log record?
The audit log captures every change to a document—when it was created, renamed, edited (including the specific field name), completed, signed, uncompleted, and when an addendum was added.
Can I see who signed a document?
Yes. If a document has been signed, a Signed By entry appears in the audit trail summary at the bottom of the document.
Does the log show what the field said before it was changed?
No. The audit log records the field name that was changed, the staff member, and the timestamp—it does not display the previous or updated field content.
