Main Problem/DX List: Track Patient Diagnoses

In Instinct EMR, the Main Problem/DX (MPL) is a running list of a patient's active and resolved diagnoses. Staff can add, edit, resolve, reorder, and void problems directly from the patient chart.

Add a problem or diagnosis

  1. Open the patient chart and go to the Visits or Snapshot tab.

  2. In the Main Problem/Dx section, select the + icon in the top right corner of the section.

  3. Enter the problem or diagnosis.

  4. Select Save.

Tip: When a problem is added to a checked-in patient, it appears within the current visit card. When added to a checked-out patient, it appears within the last active visit card.

Manage problems on the list

Select the 3-dot menu next to any diagnosis to:

  • Mark Resolved—moves the item to the bottom of the list with a checkmark (✓)

  • Edit—update the diagnosis name or details

  • Void—permanently removes the item from the list

To reorder the list, use the drag handle (slider icon) to the left of any diagnosis to drag and drop it into a new position.

Add the Main Problem/DX list to a chart document using a snippet

The MPL can be inserted into any Charts template using the snippet trigger {{MPL. Two options are available:

  • MPL - Active—inserts only active (unresolved) problems

  • MPL - All—inserts all problems, including resolved ones

To open the snippet menu, type {{ in a chart document or press Ctrl+S.

Frequently asked questions

Can I see when a problem was last updated?

Yes. Hover over the edit icon next to a diagnosis to see the last updated timestamp.

Is the Main Problem/DX list shared across visits?

Yes. The MPL is part of the patient's medical record and persists across all visits.
