Add a historical vaccine
Open the patient chart and go to the Snapshot tab.
In the Vaccinations section, select the 3-dot menu in the table header.
Select Add Vaccine History.
Enter the Vaccine Name, Practice Name, and Date Given—these fields are required.
Optionally, select a reminder label, valid for date, next due date, reminder notes, and/or notes.
Select Save.
The historical vaccine appears in the Vaccinations list with a distinct icon. Hover over the icon to see the name of the practice where the vaccine was administered.
Note: Historical records are not tied to a specific Instinct location—they are visible across your account regardless of which location is selected. Certificates cannot be generated for historical vaccines.
Add a historical health service
Open the patient chart and go to the Snapshot tab.
In the Health Services section, select the 3-dot menu in the table header.
Select Add Health Service History.
Enter the Health Service Name, Practice Name, and Date Given—these fields are required.
Optionally, select a reminder label, valid for date, next due date, reminder notes, and/or notes.
Select Save.
The historical health service appears in the Health Services list with a distinct icon. Hover over the icon to see the practice name.
Void a historical record
To remove a historical vaccine or health service:
Select the 3-dot menu next to the historical record.
Select Void Vaccination History or Void Health Service History.
Confirm by selecting Void.
Warning: Voiding a historical record is permanent and cannot be undone.
Frequently asked questions
Can I edit a historical vaccine or health service after saving it?
No. If a record was saved with incorrect information, void it and re-enter it with the correct details.
Will historical records show up in reminders?
Yes, if a reminder label and next due date were entered when the record was added.
